Event Authorization Form (EAF)

PLEASE be sure to read the new EAF Instructions at the link below as there are changes on the information required to submit your EAF.  After submission you and your Regional Director will receive email notifications the same as before.  Your Regional Director is responsible for Approving events and making any changes including Cancelling events.  The Approval and changes will immediately be reflected in the Events listings on the Regional pages.

** If you need to make a change to an already submitted event request DO NOT fill out a new request, please email your Regional Director regarding the changes. **
